Best Practice Spotlight Organization (BPSO)

What is a Best Practice Spotlight Organization (BPSO)?

Best Practice Spotlight Organizations (BPSOs) are health-care and academic organizations selected by the Registered Nurses’ Association of Ontario (RNAO) through a request for proposals process to implement and evaluate the RNAO’s best practice guidelines.

The RNAO is a professional body that is well recognized in the Ontario nursing community for their advocacy work in healthy public policy, promotion of excellence in nursing practice and providing nurses with a voice to influence and shape public policy (RNAO, 2023) . RNAO membership is currently more than 50,000 nurses and this includes a strong nursing student voice.

Since 1999, RNAO has been leading a program of evidenced based guidelines development called BPGs (Best Practice Guidelines) which led to the development and implementation in 2003 of the Best Practice Spotlight Organization (BPSO) designation program. This initiative is a key knowledge translation strategy and funded by the Ontario provincial government. Currently there are more than 1500 BPSO’s provincially, nationally, and internationally (RNAO, 2023) . Through evaluation and use of Nursing Quality Indicators for Reporting and Evaluation® (NQuIRE®) the RNAO has demonstrated that use of BPGs has improved patient outcomes. Some examples include increased care coordination and decreased falls, both important ROPs for accreditation.

The BPSO designation enriches the work environment and professional practice for nurses and other health-care providers. (RNAO, 2022) This can be a key retention and recruitment strategy, as it aligns with our staff engagement feedback from nurses who highlighted their connection to our BPSO designation. It is also a desirable feature highlighted by nursing students when on placement at Women’s College Hospital (WCH).


Revolutionized WCH BPG program and joint initiative with QSPE

Women’s College Hospital received the BPSO designation in 2015, after meeting the initial requirements which included the implementation of 3 BPGs, required 15% of nursing staff to have completed BPG champion training and engagement by the CNE in the ongoing review of BPG evidence. The program was managed with the resources available at the time and utilized a targeted approach to implement the required 2 new BPGs every 3 years, culminating in 7 BPGs by 2020. In 2021, with the change in staffing in professional practice, a base of 10 BPG champions (9% of nursing FTE) and 6 BPGs (1 having been retired) we assessed our status and our commitment to retain our BPSO designation.

The mandate of the Professional Program is to provide strong practice leadership, as this is linked to patient satisfaction and positive patient outcomes. (Wong C. , 2015) (Wong C. G., 2013) After conducting an environmental scan and a corporate needs assessment, the Professional Practice team identified a strong organizational desire to remain a BPSO site. We built on this desire and ensured our utilization of the BPG/BPSO work would align and support the 4 pillars of WCH strategic plan.

In recognition of resource limitations, the Professional Practice portfolio partnered with Quality, Safety and Patient Experience portfolio to utilize existing initiatives, programs, and systems to build a platform to support continuous quality improvement. The Revolutionized BPG program has a multi-year plan to build internal capacity and create leadership opportunities for staff interested in quality improvement at the patient, clinic, and hospital system level. 

Year 1 exceeded our expectations, with 15 QI projects that involve staff from 17 clinical programs, with a goal of submitting a poster for Academic Day 2023. We increased our BPG champions team by 650% from 10 staff to 83 (33% of nursing FTE).

At the end of our 2nd cohort, we saw a total of 30 projects in various stages of completion, utilizing 8 BPGs across approximately 60% of the organization. We have successfully maintained our BPG champion ratio of more then 15 % of nursing FTE as certified champions, with 33% of nurses at WCH holding that certification. As projects are completed, staff are submitting abstracts to various conferences and symposiums. 

Enrollment for cohort #3 is currently underway.
